• Please print and sign the consent and health questionnaire by Clicking This Link. (Spanish Form Click Here) This form MUST be filled out, signed, and brought in for every appointment.
• Please text us when you are in the parking lot to let us know that you have arrived. If you feel your child is old enough to come in alone, that is preferred in order to minimize the number of people in the office. Otherwise, we ask that you (or the guardian) walk them in but kindly wait in your car during their appointment. Our text number is 925-948-8672.
• We will be taking every patient’s temperature with a “no-touch” thermal forehead scanner.
• Every patient should brush their teeth before coming to the office (tooth brushing sinks may no longer be used due to new regulations).
• Every patient must use hand sanitizer when they enter the office.
• Everyone is required to wear a mask while in the office except during their procedure.
• Once the appointment has been completed, we will call you to update you on your child’s progress and schedule their next appointment before walking your child back out to the parking lot.
