Steps Towards Your Complimentary Smile Consult
How this works
After completing the form below and following the steps indicated, you will submit 8 sample photos of your face, teeth and mouth to us. To take the appropriate photos, please watch our video. It gives instructions on how to create the best pictures. Please look at our sample photos and video below. We recommend using two tablespoons to retract your cheeks so that you can get as close as possible to your teeth. Please hold your camera or phone horizontally while still showing the whole mouth. It is important that the photos are clear and detailed.After this is done, you will be directed to our calendar to schedule an appointment to speak with our new patient coordinator. Our coordinator will gather additional information that may be needed, and you will be given a day and time for your virtual exam consultation. At the consultation, you will be speaking with one of the doctors or with one of our treatment coordinators and they will be able to give you a preliminary treatment plan and estimate.

We also are required to inform you about the Health Insurance Portability and Accountability Act of 1996 (HIPAA) which was established for your health information privacy and security protection. Please note that when you send us your pictures via email, it is not encrypted. It is important for you to know that the guidelines of HIPAA state that if a patient or parent has been made aware of the risks of unencrypted emails and that the patient or parent provides consent to receive health information via email, then a health entity may send that patient or parent personal medical information via unencrypted email.
By sending us a request for a virtual orthodontic consultation via email you are accepting the terms and conditions of utilizing unencrypted email to communicate health information. Please be aware that all of your information stored in our computer system is encrypted after we receive it.
